Whether the shoulders sit forward or backward relative to the body, at the shoulder edge where the deltoid muscle sits.
On a try-on
Forward shoulders make a ridge or fold behind the collar and on top of the shoulder, often with a collar break, and extra fabric can show on the front panel at the armhole.
Backward shoulders pull the fabric toward the back and press against the front of the shoulder, which can also break the collar.
Without a try-on
Watch the client from the side in their natural stance. Many people today carry the shoulders forward from desk and screen habits. Clients who are active and stand well tend to sit in the regular to backward range.

Garments it affects
Jacket and shirt. It changes the position of the shoulder on the finished garment.
When in doubt
Without a fitted garment this is approximate, so a try-on is the reliable check. Default to Regular.
